Palliative or end of life care is support for people who are in the last months or years of their life, all people approaching the end of life are entitled to high-quality care and many choose to receive this care at home. Our aim is to help you to live well at home for as long as possible with dignity and respect.
You have the right to express your wishes about the care you receive, we talk with you about your wishes and preferences, we will listen to your family, friends and others important to you. Working together and with your consent we then plan care and support best suited to your needs and wishes. You plan will provide for dignified compassionate care at a level that is not over intrusive but enough to help you to manage your condition at home.
To ensure services are integrated we work alongside your Doctor, end of life nursing team and other professionals. Good communication ensures we are informed about changes to your condition allowing us to adjust you care as necessary and assisting our care teams to provide appropriate practical care and support.

Nutrition and Hydration
Assisting to preparation food and drinks, keeping you hydrated and nourished.
Medication Support
Working with health professionals by administered prescribed medication.
Domestic duties
Keeping your home clean and tidy, laundering and changing bed linen.
